DESCRIPTION OF THE RESEARCH
You are invited to participate in a research study about interactive narrative health intervention. This study is being conducted to understand the relationship between story and health behavior. Participating in this study will help us learn more about how people process and respond to flu vaccination stories. You can participate in this study on a computer, tablet, or cell phone. However, you will not be able to see the survey in Internet Explorer web browser. You must be 18 or over to participate in this study.

WHAT WILL MY PARTICIPATION INVOLVE?
You will first answer some questions (11 pages) about yourself. Then you will be exposed to one of several stories about flu vaccination. Each story scene consists of a video with subtitles and a script. However, the content is the same, so you can either watch the video (totally about 23 minutes long) or read the script (totally about 3800 words long) based on your preference. Afterward, you will answer some questions (14 pages) about your experience while watching or reading the story and some more questions about yourself. The entire study should take about 40 minutes.

How to complete the survey?
1) Complete the survey in the order of ‘Step 1 → Step 2 → Step 3’.
2) [Moving within a step] Use the ‘Previous’, ‘Next’, or ‘Submit’ black button while responding to questions within a certain step.
3) [Moving to a different step] Use the ‘Next Step’ blue button after responding to a certain step and go to the next step.
4) Complete the survey at a time. Incompletion or withdrawal of the survey will not be eligible for any compensation. Abnormal activities, such as clicking a web browser button (‘Forward’, ‘Backward’, ‘Refresh’, ‘Home’, etc.) to force the movement, deleting cookies, or closing a webpage while responding to the survey, may cause system errors, and you cannot be compensated.

ARE THERE ANY RISKS TO ME?
There is a minimal risk of a breach of confidentiality.
If you participate in this study to get extra credit in your class as a student, you need to input your own participant ID that we provided you after getting your name, student ID, class number, and email address. This information is only being collected to provide you extra credit. However, we will destroy identifiable information after extra credit is awarded.
If you participate in this study through Dynata, we are not asking for any identifiable information in this study, such as your name or email address.

ARE THERE ANY BENEFITS TO ME?
There are no direct benefits to your participation in this study.

WILL I BE COMPENSATED FOR MY PARTICIPATION?
If you participate in this study to get extra credit in your class as a student, you will receive a standard amount of extra credit after completing the survey.
If you participate in this study through Dynata, you will be compensated for this study per your agreement with Dynata.

HOW WILL MY CONFIDENTIALITY BE PROTECTED?
In extra credit participants, the data will be aggregated and analyzed with standard statistical techniques. Individually identifying data and actual study data will be stored and managed separately. We will connect them with a participant ID, and this process is only for verifying that participants should get extra credit. We will destroy identifiable information after extra credit is awarded. All data is password-protected and transmitted via an encrypted connection.
In Dynata participants, no individually identifying data will be collected as a part of this study. The research record is password-protected, and all data collected during this study is transmitted via an encrypted connection.
While there will probably be publications as a result of this study, only group characteristics will be published.
